Abstract

Understanding the psychodynamics of madness is essential to the therapy of most patients, including those who are not diagnosed as mad in the literal sense. This volume draws on Freud, Jung, recent object relation and self psychologies, and, particularly, the work of Winnicott, Bion and Elkin. It describes and critiques the basic ideas on the dynamics of psychoses and provides a framework for interpretation.

Abstract

Psychosis has been the central subject of psychiatric research for more than a century and yet it remains an intriguing enigma. This volume reviews the current status of research on psychosis in three different aspects, namely, phenomenology, which is the philosophical/conceptual basis of psychosis; psychopathology, which is the clinical manifestations of psychosis; and pathophysiology, which is the scientific pursuit for the mechanism of psychosis. Chapters focus on schizophrenia, covering such topics as clinical staging, negative symptoms, epigenetics, DNA methyltransferases, and more.

"'Franco De Masi is well known for his psychoanalytic work with patients suffering psychotic illnesses. In this book, he addresses the human vulnerability to psychosis, but the modest title of his book belies the depth of its investigations and conclusions. De Masi invites the reader into a thoughtful, systematic exploration of many aspects of the complex problems associated with psychotic illnesses: its ontogenesis and the emotional crises that lead to the dominance of psychotic thinking, the function of psychosis with regard to reality, its eruption or progression (depending upon the type of psychosis involved) and, crucially, the difficult and painstaking task of treatment. This latter theme is explored in considerable detail and is perhaps the most telling message of this book. Example after example of de Masi's engagement with patients illuminates his central objective - the gradual disinvestment by the patient of psychic energy allocated to ostensibly protective, but ultimately self-destructive, psychotic constructions and a re-investment in neurotic or more normal psychic reality - a world the patient has had to largely forsake under the sway of delusion ... Abstract

Telling Stories? explores the contemporary state of affairs in the understanding and treatment of psychosis. An inclusive approach to mental distress requires that in order to truly understand psychosis we must begin by listening to those who know this from the inside out; the voices and narrative of those who have been condemned as "unanalysable" and mad. Far from being fantastical, the complex stories that are being articulated communicate painful truths and the myriad ways in which the human psyche survives overwhelming trauma. This book is the culmination of an integrated and creative alliance between those on the cutting edge, experientially, in research, diagnosis, and treatment; this multidisciplinary dialogue proposes a new relational and attachment orientated paradigm for the 21st century. In contrast to the containment model that is currently favoured, this advocates listening and talking therapies, and the healing power of a loving relationship, offering those with psychosis the possibility of more nourishing engagement with the world.

Abstract

A distillation of many years' work on a therapeutic milieu ward of the Maudsley Hospital, in which psychotic patients were treated with an integral combination of psychiatric and psychological care anchored in the use of advanced psychoanalytic concepts of psychosis. Compelling clinical material is reproduced to help illuminate the meaning of illnesses such as paranoid schizophrenia, catatonia, psychotic anorexia and manic-depression. Several depth interviews by the author, an authority on the application of psycho-analytic thought to the problems of psychosis are reproduced for the first time.
